Efficacy of a Unani Formulation in Subjective Complaints of Shiqaqe Maqad (Anal Fissure): An Observational Clinical Study

Abstract

An anal fissure is a longitudinal split in the anoderm, with surrounding edema and inflammatory induration. Passing of hard stools has been the most common etiological factor. It affects more than 10% of patients attending proctology clinics and the incidence is growing high day by day. Almost all Unani physicians in their classical text books have mentioned anal fissure as Shigaaf in anal canal caused by various reasons among which most common cause is Sue Mizaj Har Yabis of Maqad and passing of dry hard stool. Several Unani formulations are written in classical texts for the treatment of anal fissures but the efficacy of these drugs has not been evaluated and documented yet. Therefore, it was envisaged to investigate clinically an Unani formulation as topical application mentioned in Alqanoon by Ibn Sina special reference to Shiqaqe Maqad on scientific parameters, so that an inexpensive and effective conservative treatment can be established for the management of anal fissure. This study was open labelled, observational clinical study. Thirty diagnosed subjects of anal fissure who fulfilled the protocol criteria were selected for the study. Patients were given Unani formulations topical application with dietary modification after taking their written and informed consent as per the study protocol. The study concluded that the test drug is effective in anal fissure. There was significant relief in the subjective parameters; (pain, bleeding and itching with p-values<0.001). Our study suggests that the classical formulation used as a topical application could be a better option for anal fissure in terms of efficacy.
